    Sexual Abuse Survivor Survey

The voices of survivors of sexual violence are critical to driving our efforts and shaping our work to determine the most effective, respectful, healing community response into the areas of prevention, protection and prosecution. The purpose of these questions is to learn from survivors of sexual assault and abuse how we as a community could have responded better to them, in order to assure that as we move forward, services and support are provided in ways that provide the best outcome for survivors. Your feedback will be compiled into a report and anonymously shared with the Mayor’s office to help improve the response to these crimes.

Feel free to answer whatever questions you would like, stop at any point, and comment in any manner.  All information will be kept anonymous and used in a final report.  Supportive services are offered at any time during and after you provide feedback.  Please contact 815-636-9811 (24 hours).  Thank you for sharing your time and experience with us, we appreciate your input into how we do our work.
